5-MeO-DMT Therapy protocol

Compound: 5-MeO-DMT

Status: research

Powerful short-acting psychedelic for existential exploration

Dosing

Dose: Low single-digit to low double-digit milligrams; potency varies sharply by source and method

Route: Vaporized, documented in research and naturalistic reports

Duration: 20 to 40 minutes

Sessions: Research settings only

Safety

Monitoring: Sober facilitator present throughout, recovery position awareness, no re-dosing during a session, follow-up contact in the days after: delayed reactivations are reported.

Side effects

  • Complete loss of ordinary awareness during peak
  • Physical release effects including vocalization and movement
  • Reactivation experiences in following days
  • Difficult integration when the experience outruns the person's framework

Contraindications

  • MAOI use, including ayahuasca in the same period: risk of death
  • Psychosis or mania history
  • Significant cardiovascular disease
  • Pregnancy

Overview

Mechanism: 5-MeO-DMT is a fast-acting tryptamine, distinct from N,N-DMT, producing an intense, often nondual experience with comparatively little visual imagery. Effects onset in seconds when vaporized and typically resolve within 20 to 40 minutes.

Indications

  • Consciousness research
  • Under clinical investigation for treatment-resistant depression

Evidence level: Early clinical development. Intranasal and inhaled formulations are in company-sponsored trials for depression. Naturalistic studies report large acute effects and meaningful after-effects; controlled evidence is still limited.

Integration

Prompts

  • What remains in the body?
  • What, if anything, can be said at all?
  • Any reactivations since the session?

Framework: Because content is often nonvisual and nonverbal, integration focuses on somatic and emotional after-effects rather than imagery. Voice log immediately even if words feel inadequate.

Preparation

Screening

  • Psychiatric screening including psychosis and mania history
  • Cardiovascular screening
  • Absolute review for MAOI use: combining 5-MeO-DMT with MAOIs has been fatal
  • Experienced, sober facilitation with airway-safe positioning

Set setting: Lying down in a safe, padded space with a trained sitter. The intensity and rapid onset make standing or seated positions unsafe.

Legal status: 5-MeO-DMT is a Schedule I controlled substance in the United States and controlled in many countries. Synthetic material is strongly preferred over toad-derived material for conservation and ethical reasons. Reference material, not medical advice.
